Actually you rotate once for each goal scored. So in this case where Team A scores both, they would roto twice, once for each goal scored. The roto is not on the fly, meaning that the rotations occur after each round. If during a round the ball comes off the table and needs to be reserved, it is still technically the same round so no rotation occurs until that ball has been played. This is different than a team scoring and stopping the ball as they have chosen to stop the round and start a new one, in which case the roto would happen.
